Volunteer Screening Committee Role Descriptions
This policy (Policy 1 Roles) gives a description of the roles of the Volunteer Selection Committee and the resolution committee.

Volunteer Code of Conduct
This document outlines the position of the Optmist Club of Oakridge Acres Inc and the Oakridge Optimist Baseball Association in relation to a Code of Conduct (Policy 2) for any participant of any program offered by the Oakridge Optimist Baseball Association. The document addresses policy on harassment and abuse, misconduct, inappropriate behaviour and outlines the complaint, sanction and or resolution process.

Volunteers with Criminal Records
This document (Policy 3) outlines the process in which a person may apply and might participate in our program having been previously convicted of a criminal offence.

This document outlines the process for Volunteer Screening (Policy 4) as set up by the Optimist Club of Oakridge Acres Inc.

Volunteer Police Check Form
Follow the Link and access the Background Checks tab on the London Police Services website.  Complete and submit the "For Individual Applicant Use Only - Police Records Check and Vulnerable Position Screening Application" form. If you have submitted a Volunteer Police Check form with Oakridge Baseball within the past 4 years, please complete and submit an Attestation form instead.

Volunteer Letter for Police Check
This is the letter that the London Police Service will ask for to prove that you are applying for the check as a requirement for your position as a volunteer with the Oakridge Optimist Baseball Association.

Fund Raising

Fundraising can be used for team expenses such as, but limited to, tournament fees, banquets, team social trips and events, year-end team parties and memorabilia items for players. Fundraising can also be used to purchase specific equipment items for each player such as team jackets, team bags, shirts or anything else that each player would benefit from directly.


OBA Injury Report Form
This report is designed to establish an immediate record of any incident to assist all parties in the preparation of any documents required to explain or support the incident, accident or claim referred to herein. While not all information may be required on this form in all cases, it provides a guideline for gathering information. Any incidents must be reported within 24 hours of the occurrence to the Oakridge Optimist Baseball Association.

All Sport Insurance Athletic Claim Form
This form is used to submit an insurance claim to Pearson-Dunn Insurance Brokers Inc. The OBA Incident Form (above) is filled out which gathers information for the submission of this insurance claim, if required.

Further request for information or question should contact the names provided on this form.

All insurance claims must also be forwarded to the Executive Committee of the Oakridge Optimist Baseball Association.
